K9 vest application documents are now accepted through this portal ONLY. We can no longer accept emails with attachments.

You may submit documents and images from your computer or mobile device.

You may save this application upload and come back to complete at any time within 15 days. After 15 days, all information is deleted and you will have to start over.

BEFORE YOU START: Please ensure all documents are filled out correctly and completely and have the appropriate signatures. Incomplete documents will not be accepted.

Only one proof (or training) document is required for each K9 regardless if multiple certifications are listed.
Proof must show that the K9 team(s) applying for a vest is certified together as a team and actively employed by the department and must be signed by trainer.

SAMPLE: K9 Onyx is a 2 year old Belgian Malinois certified in Patrol and Narcotics work. He is assigned to Officer John Johnson of the Phoenix (AZ) Police Department. K9 Onyx became a member of the P.P.D. in May 2012 and began his formal police training in June 2012. He has completed over 700 hours of training. Onyx likes going for walks, playing ball, chewing on his Kong, and training days. He has a strong dislike for baths and garden hoses.

5. & 6. K9 Photos
Required for the application are two photos for each K9
1. On-Duty Photo: Please provide a good quality photo of the K9 with their handler and/or vehicle. K9 must not be wearing anything other than a standard collar. No tracking harness.
2. Off-Duty Photo: Please provide a good quality photo of the K9 having some fun off the job.
We cannot accept any action shots, photos that include children, or pictures with anything in the K9's mouth
